Birth Certificates
Please read all of the information on this page before requesting a birth certificate from Hamilton County Public Health. Our office does not have birth records for persons born at hospitals located within the city limits of Cincinnati.
Hamilton County Public Health issues birth certificates for births that occurred from 1875 – present at one of the following hospitals in Hamilton County:
Bethesda North
Jewish Kenwood
Our Lady of Mercy - Anderson
Our Lady of Mercy - Mariemont
Maple Knoll
Birth records are also available for individuals born in a home located in Hamilton County, but outside the city limits of Cincinnati or Norwood.
New birth records are available eight to 10 weeks after the date of birth.
If you were not born at one of the above facilities, we do not have your birth record. Depending on where you were born in Hamilton County, one of the following health departments may have your record. It is best to call first to confirm that your record is available.

How to request a certified copy of a birth certificate
Certified copies of birth records are $19.00 and may be obtained in person, by mail or online (additional fee for this service). **As of October 16, 2009, Ohio Department of Health has instituted a mandatory fee increase. Birth records will be $24 as of this date**
By Mail
Send the following items to the Hamilton County Public Health, 250 William Howard Taft Road, 2nd Floor, Cincinnati, Ohio 45219:
- Completed application (see Download at right).
- Check or money order (no cash) payable to the Hamilton County Public Health. Certified copies of birth records are $19.00 each. *$24 as of October 16*
- Self-addressed, stamped, business sized return envelope.
In person
Bring the following items to the Hamilton County Public Health(directions):
- Payment - certified copies of birth records are $19.00 each. *$24 as of October 16* Cash, checks and money orders are accepted at the walk-up counter.
Online
Request a copy of a certified birth record online using VitalChek. There is an additional fee to use this service, but you can choose to receive your certificate in as few as 2-3 business days.
Certificate Corrections
Once a death/birth record is filed, an affidavit is required to change or correct any information on the certificate. The original document will not change - the attached affidavit will indicate the change/correction. An individual must go through Hamilton County Probate Court to make the change or correction to the original record.
