The Ohio Department of Health recently adopted a central issuance policy.
Birth records are also available for individuals born in a home located in the State of Ohio. New birth records are available eight to 10 weeks after the date of birth. Birth records are available for births that occurred from 1875 - present.
Certified copies of birth records are $24.00 and may be obtained in person, by mail or online (additional fee for this service).

- Completed application (see Download at right).
- Check or money order (no cash) payable to Hamilton County Public Health. Certified copies of birth records are $24.00 each.
- Self-addressed, stamped, business sized return envelope.

- Payment - certified copies of birth records are $24.00 each. Cash, checks and money orders are accepted at the walk-up counter. Visa, MasterCard, American Express and Discover credit cards are accepted.
Ohio Birth Certificate Corrections
Once a death certificate is filed, the funeral home listed on the death certificate can submit an affidavit to make a correction on the death record. Any changes made to a death certificate are not reflected on the original certificate. The affidavit showing the corrected information is attached to the death certificate.
Once a birth certificate is filed, corrections can be made by the individual (or parent/guardian) by submitting a birth affidavit. Once the affidavit has been accepted by the Ohio Department of Health, the birth certificate that is issued will reflect the requested correction. No affidavit will be attached. Only corrections can be processed through an affidavit. All requested changes (i.e. name changes) must be submitted to probate court. Changes accepted through probate court will also be reflected on the birth certificate that is issued. No affidavit will be attached.
