Certificates Prior to 1908

Are you requesting a death certificate for someone born before 1908? If so, you might be searching for a record that does not exist. Healthcare facilities and funeral homes were not required to file an Ohio death certificate until December 1908. Please call us at (513) 946-7800 or email us at  to find out if we have the record you’d like to order. .

Certificate Corrections
If a correction must be made to a death certificate after it has been filed, a Death Affidavit or Supplementary Medical Certification must be filed. Contact the funeral home that facilitated the burial or cremation to request the changes.

Veteran’s Death Certificate
The family of a veteran is entitled to one free copy of that veteran’s death certificate. A VA application must be submitted by either the funeral home or the Veteran’s Administration Office.

If you would like to relocate a person’s remains, you can request permission to do so from probate court. For more information on the disinterment process in Hamilton County, visit the Hamilton County Probate Court Website.
